Covid Alert Signs needed at all Rallies


ATTENTION all Dogwood Chapter Members


 

The FMCA Board of Directors voted to require Covid Alert signs, in lieu of requiring signed Covid waivers, at all FMCA functions.  Please note that all attendees are assuming their own risk (for Covid) by attending any FMCA event.  The use of safety masks is optional.

 

Rally Hosts:  Covid Alert signs need to be posted, at Dogwood rallies, where all attendees can see them.  A Covid Alert sign template has been posted on the Dogwood blog for you to access and print if you would like to use it.

President Message, October 9, 2020 Covid 19 Rally Form, Update

 Hello Dogwood Family,

 

Hope all is well and that you are all safe and healthy. The information that follows is very important, so please read carefully and comply if you plan on attending a FMCA Chapter Rally.

 

During the Virtual EAMA Annual Board meeting on Oct. 3, 2020, we were made aware of a FMCA Chapter Waiver and Release of Liability for Exposure to Covid-19 Form. If you plan to attend a chapter rally, you must agree to comply with the requirements in the form and provide a release of liability by signing the form.

 

The form is attached to this email for your review and use 

 

In order to ensure we comply with the requirements, you can do one of two things if you are planning to attend a Dogwood Rally.

 

1. Print off a copy for each individual involved, complete and sign the form, and bring it to the rally.

 

2. Complete and sign the form at the rally. We will ensure that there are sufficient copies for all attendees.

 

And just to clarify again, you must complete this form in order to participate in a rally. Your cooperation in this process will be greatly appreciated. We will get through this.

Covid 19 Rally Form, FMCA Chapter Waiver and Release

                               FMCA CHAPTER WAIVER AND RELEASE OF 

LIABILITY FOR EXPOSURE TO COVID-19



The FMCA Chapter listed below (“Chapter”) is scheduling a rally of Chapter members and guests on the date listed below (the “Rally”).  The undersigned individual (“Attendee”) wishes to attend the Rally and participate in the planned activities.  As a condition of being able to attend the Rally, the Attendee acknowledges the warnings below, agrees to comply with the requirements set forth below, and provides a release of liability as set forth below.  


  1. Adherence to Social Distancing Practices.  As a condition of attending the Rally, the Attendee agrees to comply with applicable social distancing requirements that will be distributed to attendees of the Rally.  Attendee further agrees to follow the instruction of Rally coordinators regarding interaction with others and safe distancing practices.   


  1. Prohibitions on Attendance.  Attendee acknowledges and agrees that no person may enter or attend the Rally if that person has or or has had in the past fourteen (14) days a cough, fever, shortness of breath, difficulty breathing, flu-like symptoms, gastrointestinal upset, or experienced a loss of taste or smell.  Attendee certifies that the Attendee does not have or have had in the past fourteen (14) days any of the above listed symptoms.


  1. Advisory.  All attendees to the Rally are advised that if they are age 65 or over or have any health risks such as asthma, heart problems, chronic lung or kidney disease, diabetes, compromised immune system, liver disease, or severe obesity, they are at a greater risk of serious illness or death if they contract COVID-19.  Any individual with any of these ailments or conditions are advised not to attend the Rally.  


  1. Assumption of Risk.  Regardless of adherence to health department requirements and recommendations as well as social distancing practices, there is a risk of COVID-19 exposure whenever an individual enters into a public space or building with other people.  By attending the Rally, the Attendee acknowledges and agrees that the Attendee is assuming the risk of COVID-19 exposures.  


  1. Release of Liability.  The Attendee hereby agrees to release and hold harmless Family Motor Coach Association, the Chapter, their respective officers, directors, employees, and agents from any and all illnesses, injuries, sickness, death, or other losses arising in any way from Attendee’s participation in the Rally, including, but not limited to, exposure to COVID-19.
